Please print and fill out this multi-page form completely and staple together. Please make sure all entries are legible and correct. You can also call Dell's Leather Works directly. It is suggested that you do so first on custom ordered items. Payment accepted by mail is a money order (preferred), certified bank check (preferred) or personal check to the correct amount. Please keep in mind that all personal checks must clear before your order can be processed. There is a $45.00 service charge on any returned checks and subject to collection agency fees. For credit card orders, please use our secure online shopping cart.

• Please note: When putting shipping insurance on your items, US Postal Service will not acknowledge the insurance coverage without a signature.  If it is stolen from your front porch, they do not take responsibility for it once the package is out of their hands.

• For sales outside the continental United States, we only accept Visa or Master Card purchases and no money orders will be accepted. The price for postage will vary for orders outside the USA. Please send email for a quote.

• Allow 1-3 weeks for delivery.

• See rate chart below for Shipping & Handling with and without Insurance - It is recommended when ordering with insurance to request a signature because the insurance may not be covered without it.

• New York orders, please add 8.25% NYS State Sales Tax. Shipping & Handling charges are taxable by NYS Law.

• Printed catalogs are available FREE upon request.

• All prices are in U.S. Dollars.

• Custom orders require 50% deposit and are non-refundable.
